Abstract

Meningoencephalitis is one of the main infectious diseases, associated with a high percentage of neurological sequelae, and even with high mortality rates. Objective: To describe an unusual case of meningoencephalitis of mixed etiology (bacterial and viral) complicated by ischemia and subarachnoid hemorrhage. Methodology: Description of the clinical case of a 76-year-old patient, with no relevant medical history, who presented it to the emergency department with fever, diarrhea, and altered level of consciousness. Laboratory tests, a cranial computed tomography scan, and a cerebrospinal fluid (CSF) study with the FilmArray molecular panel were performed. Results: The tomography showed a hyperdense middle cerebral artery sign and subarachnoid hemorrhage in the Sylvian fissure. The CSF showed pleocytosis, hypoglycorrhachia, and gram-positive diplococci; the multiplex PCR detected Streptococcus pneumoniae and human herpesvirus type 6. Despite early empirical treatment (ceftriaxone, vancomycin, ampicillin-sulbactam, acyclovir, and dexamethasone), the patient died within the first 24 hours due to refractory cardiorespiratory arrest. Conclusions: Mixed meningoencephalitis caused by Streptococcus pneumoniae and human herpesvirus type 6, associated with vascular complications such as ischemia and subarachnoid hemorrhage, represents a high mortality scenario, even with timely diagnosis and treatment.
