Description

High morbidity by asthma in Ecuador moderately reflects high prevalence, in urban poor, inappropriate management of this, limited access to primary and specialized care, and inhaling medications. There are no published guidelines for asthma treatment and control in the Ministry of Public Health. The investigation will be in children and adolescents from 8 to 17 years who go to emergency centers with asthma attacks: more taxpayer group to the loading of the disease and economic costs, but that will benefit more from an intervention. We will do a multi -center study in Hospitals in Poor Neighborhoods of Esmeraldas, Quito and Cuenca. The project will be done with a global health research group to study causes of asthma attacks and test strategies for prevention in Latin America, with pairs in the United Kingdom, Brazil and Ecuador: i) Identify modifiable risk factors for asthma attacks by prospective observational studies of adults in Salvador (Brazil) and children and adolescents, with asthma attacks, in 3 Ecuadorian cities; Ii) expand a specialized care program for asthma treatment in the Primary Health Care Network in Salvador (Brazil); And iii) configure specialized equipment for asthma treatment in public hospitals in Ecuador and a pilot study of extension in primary care, and iv) qualitative studies of barriers and opportunities for asthma control. The impact of the program on recurrence of attacks and costs for families and health systems will be evaluated to expand them at the state and national level in these countries.
